Price Levels for larger practices
PowerDiary Limitations
PowerDiary allows you to create services and filter which ones are displayed online. It let's you set client level discounts. That's it! If you have a practice that more than a handful of clinicians then you're going to want to standardise your pricing and "price levels" such as Provisional Psychologist vs Psychologist v Senior Psychologist vs Clinical Psychologist vs Senior Psychologist vs Director make a lot of sense. We hope that PowerDiary will implement a distinction whereby Clinicians are allocated a price level. Services have a default rate but also these defaults can be overridden with a different rate per price level. Then when a client has a default service item the rate they pay for that service item changes over time depending on who they see, or in fact the level of their clinician may change over time but the default service remains the same. There is also the opportunity to expand the service items "item number / code" to better reflect the training level and location to further reduce the item clutter... but that's a different structure.
Our Solution
We suggest planning for a larger structure from the outset and setting up all the items you need using the " (ProvPsy)", " (Psy)", " (ClnPsy)", " (SnrClinPsy)", " (Dir)" suffixes as a means to differentiate the same service as having different price levels.
Insurers
PowerDiary very nicely maps the "insurer" such as Medicare against clients and invoices.
PowerDiary limitations
What PowerDiary doesn't do is allocate different service items as being exclusive to a particular insurer or in fact the scheme under that insurer for example. We hope that PowerDiary will add the insurer to each service item such that when an invoice is set as Medicare for instance that the items displayed in the invoice details drop down are filtered to only the appropriate insurer eg Self / Medicare / NDIS etc.
Our Solution
In the mean time we have generated a comprehensive list of all the Medicare items including the various schemes such as better access vs Eating Disorders items and named them in a consistent format
Locations
PowerDiary does a great job of defining the location of each appointment and binding each location against the Medicare LocationID etc.
PowerDiary Limitations
What is missing is a definition of what each location "format" is - eg Consult Rooms vs Home Visit vs Telehealth vs Phone. We look forward to PowerDiary adding this distinction and then combining it into the service's "Item Number List" instead of a single item number depending on the location format.
Our Solution
Again we are forced to clutter the item list with the delivery format as the second list differentiator! Where the service is in the consult room we supress the suffice but for phone / telehealth and home visits we display it.
Price vs Rate
If all your services have discrete and unchangeable durations like "20 min appointments" then you can set the amount as the service total. But if you're inclined to let the last appointment go for an extra 20 minutes and then represent that as 1.2 hours at $X price then price is better off representing a rate per hour and for anything that is not a full hour we just alter the quantity as a ratio of the time delivered.
Naming structure and sort order
Try to get all your common items at the top of the list to save time scanning list. On the left hand side there are drag handles to resort if you like.
If you're working on your own then your services should be pretty straight forward but as more people start sharing a service list you might find that pricing or Medicare endorsements for a splitting and repetition. So you end up using initials or similar and then all of a sudden you have several dozen services and finding what you're looking for is next to impossible and annual repricing is worse.
Categories
If you are part of a multi-disciplinary practice that intends on using the client portal then you will want to take advantage of the Categories option in the service items. This will allow clients to filter their services according to say profession or stage of treatment such as "New Clients" vs "Existing Clients". These saleable categories are setup in the Setup > Custom Lists.
Our Service Items List
You're free to copy some or all of this structure or get in touch with us and we can help get you setup. The naming convention used here is Duration, Payee, Scheme, Location, Format, Delivery and Provider type. eg "Regular MBS Covid 1-10Sx Video (Psy)" is a 50min session rebateable under Medicare's Covid +10 sessions scheme delivered via video by a psychologist. We chose some of the naming to allow for better alphabetical sorting such as "regular" instead of "standard" because we wanted short consults to sort towards the bottom.
